If you change your address or telephone number please inform reception immediately to allow your records to be updated.

If you move to an address outside of the practice area your registration will cease and you will be required to register at a practice closer to your new home.

WE MUST BE ABLE TO CONTACT YOU AT ALL TIMES, CORRECT DETAILS MAY SAVE LIVES!!!!!
